The extended version of VILE, titled VILE: Exhumed, runs for approximately an hour and a half. Cadaver decided to expand the game after realizing there was more to tell. She had cut many elements from the original version due to time constraints and a lack of publisher support. However, the positive reception of the original game led her to revisit the project and add more content.

One of the most striking aspects of VILE: Exhumed is its use of gore and realistic imagery. Cadaver created much of the visual content herself, using leftover Halloween blood and experimenting with different angles and techniques. She emphasized the importance of grounding the horror in real-life experiences, rather than relying on supernatural elements. This approach makes the game feel more unsettling and relatable.

The game also draws inspiration from found footage and screen-life genres, which Cadaver enjoys.
